Onboarding and initial dose

Your first consultation establishes your protocol. We start you on a low dose to minimise side effects, set up the app, and book your first follow-up. Most patients lose 2-4 kg in the first month — but that's not the goal. The goal is to find the dose your body tolerates well.

Months 2 to 3

Titration and momentum

Months 4 to 6

Established protocol

The vast majority of side effects are manageable with the right protocol adjustments — slowing the titration, splitting doses, adjusting timing, or simple supportive interventions. What if I lose weight and just put it back on?
This is why Sustain exists. Every weight loss programme ends — except ours doesn't. When you reach your goal weight, we transition you to a microdose maintenance protocol at a lower monthly cost. The maintenance phase is what protects your results. Without it, regain is nearly guaranteed regardless of how disciplined you are.
Do I have to be on medication forever?
Most patients step down to a microdose protocol after reaching their goal — significantly lower than the active dose, at a lower monthly cost. Some patients eventually move off medication entirely; others maintain a low-dose protocol long-term. The clinical reality is that obesity is a chronic condition, and like most chronic conditions, it's managed rather than cured. We're honest about that upfront.

Holistic 360 Transformation uses modern GLP-1 medications (semaglutide, tirzepatide, or alternatives selected by Dr Hanlie based on your clinical picture) rather than HCG-based protocols. GLP-1s work at the hormone level — they reduce hunger signals and slow gastric emptying — rather than requiring strict caloric restriction. For patients who've struggled with willpower-based programmes, this is often the missing piece. That said, every patient is different, and Dr Hanlie may recommend a different approach if it's clinically more appropriate for you.

If you have a diabetes diagnosis, your medical aid may cover the GLP-1 medication itself (since GLP-1s are registered in SA for type 2 diabetes). The Holistic 360 programme fee is separate from the medication cost and is not covered by medical aid. Dr Hanlie will discuss your specific situation in the Founders' Consultation and may be able to coordinate with your scheme to optimise costs where possible.

Yes. Life happens. Patients pause for many reasons — travel, surgery, pregnancy planning, financial pressure, simply needing a break. We pause your billing and pause your protocol with you. When you're ready to return, we re-onboard you with a brief consultation and resume from where you left off. We never penalise you for life.
